• We will offer and advise on evidence-based treatments for acute and chronic illnesses. 

• We will offer nationally approved screening and health promotion services.

• We will visit patients whose illness or frailty prevents them from attending the health centre.

•  We will respect and protect confidential information. 

• We will respect the rights of patients to be fully involved in the decisions about their care.

Your Responsibilities To Us

• Please inform us if you change your name, address or telephone number.

• Please inform us as soon as possible if you are unable to keep an appointment so that it may be offered to someone in need. 

•  Please only request a home visit if you are too ill or too frail to come to the health centre.

•  Please treat the doctors, nurses and support staff with the same courtesy and respect that you would expect to receive from them.
