Travel Vaccinations
If you require any vaccinations relating to foreign travel, firstly you need to collect a travel risk assessment form from the Practice or complete our on-line assessment form below, please follow links at the bottom of this page.
Once you have completed the form, please hand back to a member of the reception team. All submitted forms will be acknowledged within two weeks of submission and further appointments booked as required. If you have not heard from a Nurse by two months prior to your travel date, please do check-in to ensure your form has been received. Please note that if you are travelling within one month, due to having to complete a full assessment or order any required vaccines, this may not leave enough time, and you could be advised to explore other options including private services.
Please note that we require one form completed for each patient travelling.

It is important to complete and return the risk assessment form as early as possible - at least 6 weeks before you travel - in order that you have full immunity prior to your travel. Any vaccines have to be ordered as they are not a stock item.
