New Patient Registration

registrationNew patients registering at the Health Centre will be asked to give certain information about their medical history at the time of registration, in order to help our clinicians whilst your medical records are transferred.

All new patients aged five years and over may request a new patient medical check. Please take advantage of this service, as it can be useful to the doctor if there is a delay in receiving your medical records from your previous GP. 

If you would like to register as a patient with the Practice, please call in and request a Registration Pack. This will include all the relevant forms needed for completion.

You will need to be registered as a patient before you can be seen by a doctor or nurse at the Practice, please bring in your completed forms before you require an appointment.

You may prefer to be seen by a specific clincian, and we will endeavour to do this wherever possible. If it is not possible, we will offer you an explanation. You may see any doctor in the practice, no matter who you are registered with.

If you have any questions with regard to registering as a patient, then please ask one of our reception staff.


Practice Boundary

Patients can register as new patients within the 'inner boundary' and existing patients who move within the 'outer boundary' may remain registered at the surgery, as specified on the map below. In exceptional circumstances it is at the discretion of the practice whether we accept patients who live outside these areas. See our Practice Boundary here
