Moving Away

Existing patients who move outside of the 'outer boundary' will be asked to register at a new Practice, closer to their new place of residence.

If you do move away, it might be helpful to request up to 1 months supply of any medication before you move.  We can provide you with a brief summary of your medical history for your new Practice, should that be required before your record is electronically transferred.
